HIV Medical Clinic (Primary Care)
Services
CrescentCare primary care team currently provides comprehensive medical management of HIV and related chronic conditions. Primary care services are aimed to maintain health through wellness education, monitoring of disease progression with lab tests (viral load and T-cell), immunizations, and referrals to other medical specialists if appropriate. Additionally, primary care provides individual interventions to those who are having difficulty with adhering to their HIV treatment.
A client may enter into care through two points of entry:
- a case management referral from CrescentCare or other agency
- through HIV testing and counseling
All clients must be screened for eligibility prior to their first appointment, in order to receive free services or services on a sliding scale.When going through the client screening process, new clients are asked to provide:
- proof of residency
- proof of income
- proof of HIV status
Primary care accepts patients with or without insurance.
Primary care clinic hours, by appointment only:
- Monday 8:30am – 5pm
- Tuesday 8:30am – 5pm
- Wednesday 8:30am – 7pm
- Thursday 8:30am – 7pm
- Friday 8:30am – 5pm
