CrescentCare offers case management to individuals and families living with HIV who also receive their medical services at CrescentCare. Our case managers provide support for accessing medications, help with health insurance (including assistance with insurance costs for those that qualify), treatment adherence support, and assistance with transportation coordination. Case managers also provide referrals to a number of other services including food access, utilities and rental assistance, peer counseling, health education, substance use treatment, mental health concerns, legal assistance, and more. Our goal for our clients is improved health outcomes and increased independence.

To access services, please talk with your provider or call our main line at 504-821-2601 in order to make an appointment to meet with our eligibility specialist for enrollment and to be scheduled with a case manager.
